Event Form

Fill this out as best as you can. It is designed to help you plan each aspect of your event. In order to receive promotion and/or any special services, this form must be filled out at least 3 months before the event date. Within 2-4 days, you can expect email(s) from Director(s) of services you might request (example: childcare or security, etc.). Contact tony@cityviewphx.com if you have any questions along the way.

What is the goal?

Baptism, Belong Groups, Join the Team Weekend, attending church next Sunday, Bible reading plan, spiritual gifts test, etc.

Just provide one number, not a range

You are responsible for purchasing and/or providing all food and beverages for the event (including water bottles).

Event Costs & Fees

Think about childcare fees, food, water bottles, venue, rentals, prizes, books & materials, games, etc.

Event Support Requests

You will receive an email from the Director of the services you request within 2-4 days of submitting this event form. Be prepared to answer their questions and clarify details.

Not all events will be promoted on a Sunday. The Communication Team will make that decision.

Tables, chairs, tablecloths, etc.

Silverware, plates, cups, napkins, chafing dishes, coolers, etc.

Explain placement of tables, chairs, TV, food table, check-in table, yard games, etc.

Slides on the screen, background music, mics, etc.?
